Union Wellness Centers

At the time of publication of this SPD, the Fund participates in the Union Wellness Centers. The Union Wellness Centers provide a variety of services that are available to you as a Participant or Beneficiary without deductibles or co-payments. Details on the services available can be found by going to www.unionwellnesscenters.com. The availability of particular services, the applicable payment terms and continued participation by the Fund in the Union Wellness Centers are all subject to change.
